PETITION FOR APPOINTMENT OF GUARDIAN
The undersigned Petitioner alleges:
1. Petitioner's residence is 4909 Palmetto Drive, Fort
Pierce, Florida, and his post office address is the same.
2. DAVID KEITH HODGES is a minor whose date of birth was
October 5, 1961, who is 16 years of age, and whose Social Security
Number is 264-69-7545. The minor's residence is 4909 Palmetto
Drive, Fort Pierce, Florida, and his post office address is the
same.
3. The nature of the minor's incapacity is minority.
4. It is necessary that a Guardian of the person and the
property be appointed for the minor.
S. The minor's natural father, Jess Willard Hodges, died

~ on March 17, 1978, and the minor's natural mother, Inez Hendricks,

; died on May 17, 1978. Copies of their death certificates are attached.
