CONSENT TO CUSTODY ~
That TOIrMY HAWKINS and ROSALIE HAWKINS, his wife, have this
date consented and agreed that JOYCE KUBERRY, 362 Smal]wood Drive,
Fort Pierce, Florida, can have the, custody of MARTHA KIM HAWKINS, a minor,
to-wi t:

That JOYCE KUBERRY shall have ali of the authority which can be ~
conferred upon her by the natural parents of MARTHA KIM HAWKINS to the end.
result that the custodian having said authority, shall have the same for all
purposes including school, hospitalization, learning, punishment, education,
in the same manner as would the natural parents for and during the period
MARTHA KIM HAWKINS is in the custody of JOYCE KUBERRY. -
