_ THIS CAUSE having come on for hearing and all partfes having received ~
proper and timely notice; the Court having heard testiuwny and/or cansidered
the pleadings, papers, affidavits and other papers filed herein, and being other~aise
fu11y and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child ROYLSTON F.DWARD DAMES, D.O.B.
11-12-77, is declared to be the legitimate child of the Defendant JAMES IDWARD
MILLER, and DOROTHY M. DAMES, the natural mother; it is further
ORDERID AND ADJUDGED that the natural mother, DOROTHY M. DAMES, shall
have custody of the said child subject to the Defendant's right of reasonable
visitation; it is further .
ORDERID AND ADJUDGID that commencing on June 15, 1978, the Defendant/ .
Father shall pay child support for and on behalf of the said child in the following
-manner: During each suainer week between June 15 and September 1 of each year
so long as the Defendant/Father is in school he shall pay $10.00 per week, plus
$2.00 statutor.y fee; _ thereaf ter~ the Defendant/Father shall pay the sum of $15.00
per week each and every week, plus $2.00 statutory fee. AII payments shall be made
in cash, money order or cashiers check. All money orders and cashiers checks shall
bear the payees name and Social Security Nwaber and shall be made payable to the
CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted montt?ly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winew~ood Boulevard,
Tallahassee, Florida, 32304. ~It is further
ORDERF.D ANID ADNDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until furttier order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in which eveat the support
payments shall thereafter be directed and payable to the aforesaid natural mother
or person having custody of the child; it is further

the father of the above-named child, the DEPARIMENT OF HEAI.IH AAID REHABILYTATIVE
3ERVICES, BUREAU OF VITAL STATISTICS,~AHENDMENT UrIIT, shall and it is hereby ordered
to amend the above-named child's birth certificate to show the above-named father's
name.
