The undersigned, as personal~representative of the above '
eatate, alleges: ~ . _
1. The decedent, SPHRAIM SALISBURY, a resident of St.
Lucie County, Florida, died on May 14, 1978, and Lettera of Ad-
ministration were issued to Petitioner on July 12, 1978.

2. Petitioner files herewith a waiver signed by the only ~
,
interested party, FRED$RICA M. SALISBURY, waiving the filing of a•
final accountinq. - ~
3. Petitioner has fully administered this estate by making
payment, settlement, or other disposition of all claims and debts
that were presented, and by paying or making provisions for the .
. payment of all taxes and expenses of administration.
~ 4. Petitioner has filed all.required estate tax returns
with ~he Internal Revenue Service and with the Department of Revenue f
of the State of Florida, and has obtained and filed with this court
evidence of the satisfaction of this estate's obligations for both
federal and Florida estate taxes, if any.
5. Petitioner purposes to make distribution of all tY~e =
assets of this estate to the sole beneficiary, FREDERICA M. SALIS-
BURY, 2516 South 13th Street, Fort Pierce, FL 33450.
~ 6. Petitioner is the only person having an interest in this
proceedinq.
WHEREFORE, Petitioner request that, after satisfactory proof
has been presented that distribution has been made in accordance
with the proposed distribution stated herein and that claims of
