Before me, the undersigned suthoritq, personally appeared VIOLA ~

MARIE CONLEY, who on oath, being duly sworn, deposes and says:
l. That she is the surviving epouse of CHARLES HENRY CONLEY,
who died on ~ ~ , 19 7~, as shown in the Certified Copy
of the Death Certificate attached hereto as EXHIBIT "A".
2. That at the time of his death, they were the owners of the
folloving described real property as Tenants by the Entirety:
Lot 10, Block 456, PORT ST. LUCIE, SECTION 10, a Subdivision according
to the Plat thereof recorded in Plat Book 12, page 49, of the public
records of St. Lucie County, Florida.
3. That the marriage of the Affiant and CHARLES HENRY CONLEY Was
~ continuous from the time they acquired title to the above described property ~
untiX his death and she has not since remarried.
4. That the estate of C~iARLES HENRY CONLEY was not liable for the
~
filing of a Federal Estate Taa Return, form 706, and that there were no
~
Estate Taxes due and owing to the State of Florida as evidenced by the
Florida Non-Taxable Certificate.recorded in Official Records Book 263, page
2561, St. Lucie County, Florida, public records.
