The undersi~;ned does her~by apply to the Tort Pierce Utiiities
Autl~ority for an extension of water service outside Lhe City Limits of
the City of.Fort Pierce, Florida, to the follo~ein~ described property

I understand that in order to obtain extension of such water
service~ that I ~vill be required to pay the cost of canstruction of
such ~vater line from thc~ ~oint of conncction ~cith exisLing lines of the
Utilities Authorit~ to my property~ that the total of such cost shall
be deposited with the Utilities AuthoritS~ prior to the time of the
construction of said line~ that I ct~ill be obligated to pay all meter
fees and connectioi~ charges rec~uired under the Resolutions af the
Utilities Authority for such service prior to the ir.stallati~n of said
1 ine, and ~hat I wiTl p~y, t~~:~en billed, for all ~vater furnishecl to my
property according to the rates established b~~ the Utilities Authority
for t?~e service I receive.
In addition to the deposit of said construction cost as set
forth in the a~ve para~rapt~, I understand that upon execution of this
application, I am required to depasit ~~~it-h the Utilities Authority an
amount equal to ten (lOd) percent of the connection fees for ~cater
service for each unit to be served to and including t~venty-five (25)
units, and five (5p)~percent of such connection fees for each unit_in
excess of twenty-five (25) units. In the event this applicati~n is not
approved by the Utilities Authority and the City Commissic~n ~r the City
of Fort Pierce, then such deposit ~vill be returned to me. If tn~s
application is ~approved by the Utilities Authority and the Cit1~ Ccrr~ission
of the City of Fort Pierce; the depc~sit_of the connection fees as above
outlined shall be.credited to the total cost of the connec~ion fees.
I further understand that i f this appl ication is ap~>>~~ve~i bS~ the
L'tilities Authority and the City Com:nission of the City of Fort Pierce,
Flori~la, and I have not completed constructi~n of the water line requ~sted
~vithin onc year fro:n the date af the approvll b` thc City Co:nr:~ission af
the City ~f Fort Pierce, Florida, then such approval s.~all beco~ne null
and ~oid and it wil l become• necessary for r~e to i ni L iate a ne~v appl ication
for suc+.~ ~~~ater ser~~ice.
I understand that the extension of saici~rater service to my
~ronerty shall be contingent upon the abilitt~ of the Utilitie:: Autnorit~~
to supply the denand reqiiested by me ~vithout in any tvay intcrfering cvith
the ~vater services no~v perforrned by= the Utilitie5 ~ltitharity for th~
