UNDER FLORIDA WORKMEN'S COMPENSATION
ACT
COMES NOW WILLIAM L. MARTIN, d/b/a BILL MARTIN GARAGE, -
employer of the Plaintiff, GLENN R. MARTIN, and his workmen's
compensation insurance carrier, AUTO OWNERS INSURANCE COMPANY,
a foreign corporation authorized and doing business in the
State of Florida, by and through their undersigned attorneys,
pursuant to Florida Statute §440.39 (3) (a), hereby give notice
that GLENN R. MARTIN was, at the time and place he was injured
as alleged in the Complaint heretofore filed in this cause,
in the course and scope of his employment and that the employer
and insurance carrier hereinabove named have paid to the
Plaintiff, GLENN R. MARTIN, various sums of r.?oney for workmen's
~ compensation benefits, including temporary total disability,

permanent partial disability, medical benefits and remedial
benefits, and that said employer and insurance carrier give
further notice herein that future compensation anc? medical
benefits will be paid to-the Plaintiff, GLENN R. MARTIN.
WHEREFORE, [VILLIAM L. MARTIN, d/b/a BILL MARTIN GARAGE,
and AUTO OWNERS INSURANCE COMPANY, by and through their under-
- signed attorneys, pray that this Notice be filed in the above-
styled cause and be recorded by the Clerk of the Court, and

that the same, according to statute, constitute a lien upon

any recovery by the Plaintiff, GLENN R. MARTIN, for the full =
amount of the benefits previously-paid to the said Plaintiff,
