employer of VOLEY ANNUEL MARTIN, a minor, by_and through
WILLIAM L. MARTIN, as parent and next friend, and AUTO OWNERS
INSURANCE COMPP.NY, a foreign corporation authorized and doing
business in the State of Florida, by and through their undersigned
attorneys, pursuant to Florida Statute §440.39 (3) (a), hereby
give notice that VOLEY ANNUEL MARTIN was, at the time and place
he was injured as alleged in the Complaint heretofore filed in
this cause, in the course and scope .of his employment and that
the employer and insurance carrier hereinabove named have paid
f to the minor, VOLEY ANNUEL MARTIN, various sums of money for
workmen's com ensation benefits, includin tem orar total

r disability, permanent partial disability, medical benefits and

~ remedial benefits, and that said employer and insurance carrier
give further notice herein that future compensation and medical
benefits will be paid to the minor, VOLEY ANNUEL MARTIN.

WHEREFORE, WILLIAM L. MARTIN, d/b/a BILL MARTIN GARAGE,

and AUTO OWNERS INSURANCE COMPANY, by and through their under-

Y signed attorneys, pray that this Notice to filed in the above-
styled cause and~be recorded by the Clerk of the Court, and
that the same, according to statute, constitute a lien upon

any recovery by the minor, VOLEY ANNUEL MARTIN, for the full
amount of the benefits previously paid. to the said minor,
