This cause was heard upon the sworn Petition for
Adoption filed in this cause and the Answer of the Florida
Department of Health and Rehabilitative Services, and it

appearing to the court that the petitioner Jerry G. Holley `
is a fit and proper person to adopt the minor child sort to
be adopted and that the best interest of the child will be
promoted by the adoption and that the child is suitable for

adoption by petitioner, and being otherwise fully advised in
I the remises it is

ADJUDGED that the minor child known as John~William
Costello is declared to be the legal child of the petitioner '
Jerry C. Holley and is given the name of John William Holley,
I! by which name he shall hereafter be known, and that this child
be the child and legal heir of the petitioner Jerry C. Holley,

entitled to all rights and privileges and subject to all
obligations of a child born to the petitioner in lawful wedlock
r It is further adjudged that Roger N. Messer, as court
appointed attorney herein, receive the sum of $~d~ for
