The undersigned does hereby apply to tae Fort Pierce Utilities
Authority for an extension of water service outside the City Limits of
the City of Fort Pierce, Florida, to the following described property
which I own, to-~r•it

I understand that in order to obtain extension of such water
service, that I will be required to pay the cost of construction of
such water line from the point of connection with existing lines of the
Utilities Authorit~ to my property, that the total of such cost shall
be deposited with the Utilities Authority prior to the time of the
construction of said line, that I will be obligated to pay all meter
fees and connection charges required under the Resolutions of the
Utilities Authority for such service prior to the installation of said
line, and that I Nill pay, when billed, for all water furnished to my
property according to the rates established by the Utilities Authority
for t:~e service I receive.
In addition to the deposit of said construction cost as set
forth in the above paragraph, I understand that upon execution of this
application, I am required to deposit with the Utilities Authority an
amo~;nt equal to ten (l00) percent of the connection fees for Hater
servi"ce for each unit to be served to and including twenty-five (25)
unil.s, and five ( 5 0) "percent of such conne~bior,~i ,~qes for each unit in
excess of twenty-five (25) units. In the event this application is not
approved by the Utilities Authority and the .City Commission of the City
` of Fort Pierce, then such deposit will be returned to me. If this
application is approved. by they.Ut'lities Authority and the City Commission
of the City of Fort~Pieree, the s~eposit of the connection fees as above
~ outlined shall b~ credited to the total cost of the connection fees.
I further understand that if this application is approved by the
Utilities Authority and the City Commission of the City of Fort Pierce,
Florida, and I have not completed construction of the water Line requested
within one year from the date of the approval by the City Com~hission of
the City of Fort Pierce, Florida; then such approval s:iall become null
and void and it will become necessary"for me to initiate a ne~v application
for such water service.
I understand that the extension of said water service to my
property shall be contingent upon the ability. of the Utilities Authority
to supply the denand requested by me without in any=~r•ay interfering with
the water services now performed by the Utilities Authority for the
inhabitants of the City of Fort"Pierce, Florida.
g I hereby agree that in consideration of m3• request for the
extension of water service to my property under the conditions above ~j
mentioned, that if r*my property becomes"contiguous to the City Limits of ~I
