DAVID, a resident of St.~ Lucie County, Florida, died on April 27, 1978, and Letters of Administration were issued to Petitioner on June 28, 1978. - 2. Petitioner files herewith a Waiver of Accounting and Service of Petition for Discharge and Consent to Discharge of Per- sonal Representative. 3. Petitioner has fully administered this estate, by making payment, settlement, or other disposition of all claims and debts that were presented, and by paying or making provision for- -the payment of all expenses of administration. t ~ 4. Petitioner has filed all required estate tax returns t with the Internal Revenue Service and with the Department of Revenue l of the State of Florida, and has obtained-and filed with this court evidence of the satisfaction of this estate's obligations for both federal and Florida estate taxes, if any. ~ 5. -The only persons having an interest in this proceed- ing and their respective addresses are: CLAUDINE CERILLO DENSRY, 1087 N.W. 89th Avenue, Plantation, Florida, 33322, Petitioner. 6. Petitioner requests that, after satisfactory proof s has been presented that distribution has been made in accordance with the schedule of distribution and that claims of creditors have been paid or otherwise disposed of, an order be entered discharging Petitioner as personal representative of this estate and releasing the surety-on any bond which Petitioner may have posted in this FEE.
