Pursuant to Section 440.39 of the Florida Statutes, WARREN'S f.
LAUNDRY, INC., Employer of the Plaintiff, ALMA D. MINUS, and LIBERTY
MUTUAL INSURANCE COMPANY, Workmen's Compensation Carrier for the
Employer, give notice that ALMA D. MINUS, was in~ured~ in the course

of her employment with WARREN'S LAUNDRY, INC. at the time and place
alleged in the Complaint filed in this cause. Pursuant to the
Florida Workmen's Compensation Act, various sums of money for
workmen's compensation and medical benefits have been paid by the
Employer and Carrier-to or in behalf of the Plaintiff, and it is

probable that future compensation and medical benefits will be paid

WHEREFORE, WARREN'S LAUNDRY, INC., and LIBERTY MUTUAL
