PETITION FOR DISCHARGE OF GUARDIAN
The undersigned, as guardian of the person and property of
Helen Rassmussen, incompetent, alleges:
1. The need for the continuation of this guardianship no
longer exists by reason of the fact that Helen Rassmussen died
November 1, 1978, as is~evidenced by a certified copy of her death
certificate which is attached hereto and made a part hereof by
reference.
2. Petitioner files herewith a final return containing a
full and correct account of all receipts and disbursements of all the
ward's property of which petitioner has control since the last return
filed herein, if any, or from the commencement of the administration
of this guardianship if no prior return has been filed, which return

j includes a statement of the ward's assets.
3. Petitioner represents that a copy of the foregoing
has been provided to Mrs. Dorothy Timma, personal representative of
Helen Rassmussen.
WHEREFORE, petitioner requests that an order be entered
approving all accountings not previously approved and discharging
petitioner as guardian and releasing the surety on on petitioner's
bond, if any, from further liability. -
