R. PATRICK BEATTY, who, being duly sworn, says that he is the Agent
of the Lienor herein, CREATIVE HOMES BY M~ITHIAS, INC., whose address

pursuant to a contract-with ELIZABETH MERCER, 422 Willows Avenue,
Port St. Lucie, Florida 33452, Lienor furnished labor and materials
for the construction of a residence, for a total value of $8,600.00,
which remains .unpaid, for which amount a lien is claimed on the ~
following described real property in St. Lucie County, Florida: ~

Affiant further says that Lienor furnished the first of said
labor and materials on the 3lstday of October , 1978, and
the last of the same on the 5th day of January 1979. No
notice to Owner was required or given, since Lienor dealt directly
