THIS CAUSE having come on for final hearing on the 23rd day
of 1?iay, 1978, and the Respondent having been present in Court, and the
Court being advised in the premises, it is,
ORDERID ADD ADJUD(~.D that the Respondent pay to the Petitioner
for the berefit of T4~I bIARIE CUS:4'~LLI, date of birth: June 16, 1975,
.minor child of the parties, the sum of X20.00 per week, plus an additional
X2.00 for payment of Clerks fee for receiving and disbursing support payments.
These payments are to begin. on the 30th day of l~iay, 1978, and thereafter on
Tuesday of each and every week; .that said payments shall be made in cash,
by cashier's check or by postal money order to the Clerk of the Circuit
Court of St. Lucie County, P. O. Box 700, Fort Pierce, Florida, and the
said Clerk shall. transmit the said payments to: Child Support Enforcement
Section, 1317 pinewood Boulevard, Tallahassee, Florida, 32301, far transmittal

to the State ~f New York as long as the case is certified as a Title N-D
case. If the Clerk receives notification by the Department of Health-$ Re-',
habilitative Services that the case is no longer a IV-D case, he will then
forward all support~to: Suffolk County C. S. E. B./Support Collection Unit,
P. 0. Box 200, Veterans i`Iemorial Nighway, Hauppauge, Nc1~ York 11757.
ORDERED ~l~ ADJiJDGED that this tbnorable Court shall retain
~ jurisdiction of this cause and the parties hereto for the entry of such
further Orders as may be deemed necessary.
