1. Petitioners residence is Fort Pierce, St. Lucie County,
Florida and petitioners post office address is L-3 Lazy Lane,
Plantation A:anor, Fort Pierce, Florida 3345G.
2. Petitioner, whose date of birth was A:arch 13, 1930 is
48 years of age, and her Social Security number is 283-26-6879.
3. Petitioner, although mentally competent, is incapable of
the care, custody, and management of her estate by reason of age or
physical infirmity and voluntarily petitions the Court for the
appointment of a guardian of her property pursuant to the applicable
provisions of the Florida Statutes.
4. This petition is accompanied by a certificate of a licensed
physician that hP has examined the petitioner and that the petitioner
is competent to understand the nature of the guardianship and her
• delegation of authority.
5. The approximate value and description of the petitioners
property are: her interest in an Estate now being probated in the
State of Ohio the value of which Estate is estimated to be 450,000.00,
Said Estate is expected to be distributed between sixty and ninety days
from the date of this Petition, The petitioner has other personal
property of an approximate value of less than 5,000.00.
