by and through its undersigned attorneys, pursuant to Section f
440.39 gives notice that Rodger Mann was in the course of
his employment with his employer, Law's Electirc, Inc., when
a
he was injured at the time and place alleged in the Complaint

filed herein, and that said workmen's compensation carrier,
Vigilant Insurance Company, through the date of this notice
has paid to Plaintiff, RODGER D. MANN, the sum of $1,921.50
in workmen's compensation benefits and the sum of~$1,286.75 for
medical benefits, and said workmen's compensation carrier

gives further notice herein that it is 'probable that future
~ compensation benefits, medical benefits and other benefits

under the Florida Workmen's Act will be paid to Plaintiff.

WHEREFORE, Vigilant Insurance Company, by its
undersigned attorneys, prays that this notice will be recorded

and same constitute a lien upon any recovery by the Plaintiff
~ for the full amount of benefits previously paid to the
Plaintiff under the Florida Workmen's Compensation Act, and
also prays that said notice constitute a lien upon any
