PETITION FOR VOLUNTARY GUARDIANSHIP .
The undersigned petitioner alleges:
1. Petitioner's residence is 1010 Flood Road, Fort Pierce,
Florida, and petitioner's post office address is 1010 Flood Road,
Fort Pierce, Florida.
2. Petitioner, whose_date of birth was July 17, 1898, is
80 years of age, and his social security number is 372-34-4171.
3. Petitioner, although mentally competent, is incapable of
the care, custody, and management of his estate by reason of age or
physical infirmity and voluntarily petitions the court for the
appointment of a guardian of his property pursuant to the applicable
provisions of the Florida Statutes.
4. This petition is accompanied by a certificate o£ a licensed
physician that he has examined petitioner and that petitioner is
competent to understand the nature of the guardianship and his
j delegation of authority.
( 5. The approximate value and description of the petitioner's
