personal representative. Petitioner's name and address are RICHARD C.
RIDGWAY, 1706 Bayshore Drive, Fort Pierce, Florida 33450 and the name
and address of petitioner's attorney are as set forth at the end of this

2. Decedent, Keith B. Ridgway, whose last known address was
1706 Bayshore Drive, Fort Pierce, Florida 33450, and, if known, whose age
was 79 and whose social security number is 302-30-8481 died on February
28, 1979 at Lawnwood Medical Center, and at the time of death decedent

was domiciled in St. Lucie County, Florida.
3. So far as is known; the names of the beneficiaries of this
