2. Decedent, THEODORE G. RAPP, SR., whose last known address

was 2884 Treasure Island Road, Port St. Lucie, Florida, and if known,
whose age was_82 and whose social security number is 147-03-2339
died on March 18, 1979 at Martin Memorial Hospital, and at the time of
death decedent was domiciled in St. Lucie County, Florida.
