2. Michelle Renee Larkin is an incompetent, whose date
of birth was December 20, 1971, who is seven years of age and
whose social security number is being applied for. The residence

of the incompetent is 2815 Fairway Drive, Fort Pierce, Florida. - -
3. The nature of the incompetent's incapacity is that
she is -a minor.

4. It is necessary that a guardian of the person and

5. The approximate value and description of the in-

- and the proceeds from an insurance policy in the amount of $20,000.
