PETITION FOR ORDER AUTHORIZINC SETTLEMENT
Petitioner, the undersigned, as ~personal representative of the
estate of GEORGE PROGTOR, deceased, represents and shows unto the
( Court as follows:
1. The Petitioner, as personal representative of the estate of
GEORGE PROGTOR, has made claims for damages against, Ramon
Perez, Gulf Bay Sod, and his (their) liability insurance carrier,~, Home
~ Insezr~nce Company, for personal injuries sustained by the deceased from
~ an automoUile accident that occurred on 4/21/76 in Temple Terrace,
Hillsborough County, Florida. That the personal injuries suffered by
~ GEORGE PROGTOR, deceased, were a direct and proximate result of the
accident of 4/21/76 and all medical bills incurred by the deceased were
,
j paid by the insurer of the deceased.
~ 2. The Petitioner and Home Insurance Company, the insurer for
i

~ . Ramon Perez and/or Gulf Bay Sod have reached a compromise settlement
~ of the decedent's claim for said damages sustained from the automobile
e
; accident for a total sum of $2, 500. 00, consisting of the sum of $1,168.18 to
~ the Petitioner, as personal representative of the estate of GEORCE PROCI'O ,
~
~ deceased; the sum of $833. 33 to Lawrence Luppens, as attorney's fees

pursuant to a contingency fee contract for representing the Petitioner in
~
~ prosecuting the decedent's claim; the sum of $498. 49 to Lawrence Luppens
~
as court costs incurred in prosecuting the decedent's claim. The Petitioner
