The undersigned. as personal representative of the above ~

Estate, alleges: z

1. The decedent, GLEN H. ERISMAN, a resident of St. Lucie
t
County. Florida, died on October 20, 1977, and Letters of Administra- ~
€
tion were issued to Petitioner on November 14, 1977. ~
2. Petitioner files herewith a waiver signed by Petitioner,
as the sole beneficiary of this Estate, waiving the filing of a
final accounting. ~

3. Petitioner has fully administered this Estate by making `
payment, settlement, or other disposition of all claims and debts
' that were presented, and by paying or making provision for the pay-
i
ment of all taxes and expenses of administration. ;

~ ' 4. The amount of compensation either paid or to be paid to
i
~ the personal representative, attorneys, accountants, appraisers or
,
~ other agents employed by the personal representative is set forth
~ in Exhibit A attached hereto.

~ 5. Petitianer has filed all required Estate tax returns

~ with the Internal Revenue Service and with the Department of Revenue

~ .
~ of the State of Florida, and has obtained and filed with this Court
~
~
~ evidence of the satisfaction of this Estate's obligations for both

~ Federal and Florida Estate taxes, if any. ~

z ~ b. Petitioner has made or proposes to make distribution of ~

~ the assets of this Estate as set forth in the Schedule of Distribu-
~ tion attached hereto as Exhibit B. 4

~ 7. Petitioner is the only person having an interest in this ~
