BEFORE ME, the undersigned authority, personally appeared
GERALD JOHNSON, Building Contractor, by and through .his attorney,
OSBORNE WALKER O'QUINN, who, being duly sworn, says that he is the
attorney for the lienor herein whose address is 201 South 2nd Street,
Fort Pierce, Florida and that in pursuance of a contract with DOUGLAS
GILBERT and MARTHA GILBERT, his. wife, lienor furnished-labor, services
of materials on the following described real property in St. Lucie
County, Florida owned by DOUGLAS GILBERT and MARTHA GILBERT, his wife,
more particularly described as:
Lot 20, Block 11, SURFSIDE SUBDIVISION,
Unit 2, according td the Plat thereof
as recorded in Plat Book 11, at Page 8
of the Public Records of St. Lucie
County, Florida.
A total value of $3,207.00 remains unpaid and due and owing and the
last of which was furnished on or about-March 15, 1979..
