The undersigned petitioner alleges:
1. Petitioner has an interest in the above estate as wife of the deceased,
EORGE PROCTOR. The name and address of the Petitioner is as set forth
in paragraph 3 and the name and address of her attorney is as set forth at the
end of this Petition For Summary Administration.
2. Decedent, GEORGE PROCTOR, whose last known address was
2804 Avenue P, Fort Pierce, Florida, died on June 24, 1977, at Orange -
Memorial Hospital, Orlando, Florida in Orange County, and at the time
of death decedent was domiciled in St. Lucie County, Florida.
3. So far as is known, the names of the beneficiaries of this estate,

their addresses and relationships to decedent, and the ages of any who are
minors, are:
