The undersigned, as personal representative of the above
estate, alleges:
1. The decedent, JOSEPH M. STERK, a resident of Sycamore,
Illinois, died on March 14, 1977, and Letters of Administration
were issued to Petitioner on September 30, 1977.
2. Petitioner files herewith appropriate Waivers of Ac-
counting and Service of Petitions for Discharge and Consent to
Discharge as Personal Representative for each interested party
set forth in this estate.
3. Petitioner has fully adminis-tered this estate, by making
payment, settlement, or other disposition of all claims and debts
that were presented, and by paying or making provisions for the
payment of all expenses of administration.
~ 4. Petitioner has filed all required estate tax returns
with the Internal Revenue Service and with the Department of Revenue
~ of the State of Florida, and has obtained and filed with this
Court evidence of the satisfaction of this estate's obligation

for both Federal and Florida State taxes, if any. '
5. Petitioner has made or proposes to make distribution of
the assets of this estate as set forth in the Schedule of Distribu-
. tion attached hereto as Exhibit A. -
