(2) JAMES G. PEERY is an incompetent, whose date of_
birth is February 21, 1892, who is 87 years of age, and whose
Social Security number is 719-01-0849. The residence of the
incompetent is 1009 Beach Court, Fort Pierce, Florida 33450.
(3) The nature of the incompetent's incapacity is -
organic brain syndrome, senile and dementia, as adjudged
by the Circuit Court of the 19th Jud~cial Circuit, in and
for St. Lucie County on April 6, 1979.
(4) It is necessary that a guardian of the person and
~ property be appointed for the incompetent.
