PETITION TO TERMINATE GUARDIANSHIP AND
DISCHARGE GUARDIAN
COMES NOW LELA FiJLLER NEELY and petitions this Court to
terminate the voluntary guardianship of her property and to discharge
the Guardian•appointed and alleges in support thereof:
1. That previous hereto the Petitioner did sign a Petition
for Voluntary Guardianship requesting this Court to appoint a
Guardian of h~r property and that at the time of the signing of said
Petition for Voluntary Guardianship the Petitioner was in anticipation
of undergoing serious surgery.
" 2.. That at the pr.esent time, the Petitioner has undergone
~ said surgery and has successfully and satifactorily achieved

recuperation and recovery and that a Guardianship of her property
"is no longer necessary.
3. That even though an order appointing Guardian has been
entered in this cause, the Letters of Cuardianship have not been
entered:

4. That the Petitioner requests this court to terminate the
Guardianship, discharge the Guardian and that the Court order the
~ Guardian to return to the Petitioner all property of the Petitioner

~ which the Guardian may have possession of and for the Guardian to make
a full accounting to your Petitioner. ~
