TNIS CAUSE having come on for hearing on the petition of the DEPARIMENT
OF HEALTH AHD REHABILITATNE SERVICES for child support and the Court having found ~
that said Petitioner is a proper party by virtue of an assignment of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGID that the Petition for Child Su ort shall be and
it is hereby granted; it is further r
ORDERED AND ADJUDGED that commencing on 2~ , 1979,
the responsible parent GWENDOLYN LEE GREEN , who is the mo ther
of the minor child (ren) , to-wit : M~RCELLUS TERRANCE GREEN
shall pay to the Cle k of this Court the sum of $ fj l~ Uc) per each and
every , plus $2.00 statutory fee. All payments shall be made in cash,
money order or cashiers check. All money orders and cashiers checks shall bear the
payees name and Social Security Number and sha11 be made payable to the CLERK OF THE

ORDERID AND ADJUDGF.D that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit supporC payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Department
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
