THIS CAUSE having come on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERID AND ADJUDGED that the minor child, JENNIFER DAWN CAMP,
d/o/b July 13, 1978, is declared to be the legitimate child of -the Defendant
MILLARD DANIEL McBRIDE, and CHERYL ANN CAMP, the natural mother; it is further {
ORDERID AND ADJUDGID that the natural mother, CHERYL ANN CAMP, shall '
have custody of the said child subject to the Defendant's right of reasonable
visitation; it is further
ORDERID AND ADJUDGID that commencing on MAY 1, 1979, the Defendant/
Father shall pay child support for and on behalf of the said child in the amount
of $150.00 per month, plus $2.00 statutory fee, until the amount of $553.00 is paid
in full. -
1 payments shall be made in cash, move
order or cashiers check. All money orders and cashiers checks shall bear the
payees name and Social Security Number and shall be made payable to the CLERK OF
THE CIRCUIT COURT, and sent to:

SUPPORT DEPARTMENT
POST OFFICE BOX 700
FORT PIERCE, FLORIDA 33450.
Said amounts up to $553.00 shall be remitted monthly by the Clerk to the Department
of Health and Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood
~ Boulevard, Tallahassee, Florida, 32304; it is further
ORDERID AND ADJUDGID that the Clerk of the Circuit Court shall and he is
~ hereby ordered to continue to transmit support payments up to $553.00 received from
the Defendant until further order of this Court or receipt of Notice to Discontinue
~ Payments from the Department of Health and Rehabilitative Services, ./s~,~J/~.
it is further
ORDERID AND ADJUDGID that the above-named Defendant having been adjudicated
the father of the above-named child, the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered
to amend the above-named child's birth certificate to show the above-named father's
name.
