COMES NOW The Maryland Casualty Insurance Company, a
corporation, by its undersigned attorneys, and files this its
Notice of Payment of Compensation and Medical Benefits and
shows:
1. That on February 22, 1977, the date of the accident
alleged in the complaint herein, the plaintiff, RICHARD C.

HEELER, was an employee of Frank DeThomas Construction, Inc.,
and the alleged accident arose out of and in the course of his
employment. At that time The Maryland Casualty Insurance
-
Company carried the workmen's compensation insurance coverage
on the employees of Frank DeThomas Construction, Inc., which
coverage was provided in accordance with the provisions of the
Florida Workmen's Compensation Law (Florida Statutes, Chapter
I 440.01 et seq.).

~ 2. In accordance with the provisions of the compens tion
~ law referred to The Maryland Casualty Insurance Company has
through April 20, 1979, paid to Richard C. Heller the sum of

~ $6,137.40 in compensation benefits together with the sum of
$1,120.00 in medical expenses resulting from said injury.
3. That The Maryland Casualty Insurance Company is

- continuing at the present and will be required-in the future to
provide compensation and medical benefits to the plaintiff,
Richard C. Heller, all because of the-accident referred to above.
