- WHEREFORE, pursuant to the provisions in Florida Statute
440.39, The Maryland Casualty Insurance Company files this its
Notice of Payment of Compensation and Medical Benefits because
of the injuries to Richard C. Heller and hereby claims a lien
in the amount of $7,257.40 as provided by law upon any judgment
recovered by Richard C. Heller in this action.
I HEREBY CERTIFY that a copy of the foregoing Notice
of Payment of Compensation and Medical Benefits has been
furnished by mail on this 20th Day of April, 1979,. to Gerald S. z
James, Esquire, Post Office Box 3089, 211 South Second Street,
Fort Pierce, Florida 33450, as Attorney for the Plaintiff and
to the Attorneys for the Defendants when they appear. ~
