your Petitioner requests that JANET M. HOGARTH serve without bond
unless this Court in its discretion decides that a bond should be
required and that in the event that this Court does require a bond
of JANET M. HOGARTH, your Petitioner directs that the premium for
said bond be paid out of Petitioner's estate.
6. Your Petitioner also requests that this Court
order JANET M. HOGARTH to pay to BRAD CULVERHOUSE, Esquire a
reasonable attorney's fee and costs out of her estate for represent-
ing the Petitioner in this cause.
7. That Petitioner's physician is Dr. Joseph
Ilcus M. D., of Fort Pierce Florida, and his
certificate executed pursuant to Section 744.341, Florida Statutes,
is attached hereto and incorporated herein.
8. That Petitioner spec if ically requests for this
Court to act on Petitioner's Petition for Voluntary Guardianship
ex parte and Petitioner specifically waives any notice of hearing and
any and all rights to be present at said hearing and Petitioner prays
for this Court to without notice appoint JANET M. HOGARTH as Guardian
of Petitioner's property and to issue to her letters of guardianship.
