THIS CAUSB having come on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERED AND ADJUDGID that the minor child(ren)
LAGRETTA ANN BARFIELD, d.o.b. 1-1-74
is/are declared to be the legitimate child(ren) of the Defendant
RALEIGH BERNARD BARFIELD and PATRICIA ANN McGHEE
the natural mother; it is further _
. ORDERID AND ADJUDGED that the natural mother,
PATRICIA ANN MCGHEE shall have custody of the said child(ren)
subject to the Defendant's right of reasonable visitation; it is fu~irther
ORDERID AND ADJUDCID that commencing on M Q ~ ~ a 1979,
the Defendant/Father shall pay child support for and on be if of the said child(ren)
in the amount of $ ~-S . ~ ~ per Q e- plus $2.00 statutory fee. All
payments shall be made in cash, money order or cashiers check. All money orderg and
cashiers checks shall bear the payees name and Social Security Number and shall be

Said amount shall be remitted monthly by the Clerk to the Depar-L`m~t~~E~~E
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit suppo,~ct.payments received from the Defendant
until further-order of this Court or receipt of Notice to Discontinue Payments from
~ the Department of health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother

x or person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
- the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, ATIENDMENT UNIT, shall and iC is hereby ordered to:
