THIS CAUSE having come on for hearing on the petition of the DEPARTMENT !
OF HEALTH AAjD REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to ~
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is 3
ORDERID AND ADJUDGID that the Petition for Child Support shall be and
it is hereby granted; it is further ¢

the responsible parent ALBERT MORRIS ANDERSON who is the father ~

money order or cashiers check. All money orders and. cashiers checks shall bear the
payees name and Social Security Number and shall be made payable to the CLERK OF THE

Said amount shall be remitted monthly by the Clerk to the Department of He81th and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERID AND ADJUDGID that the Clerk of the Circuit Court shall and (a)he
is hereby ordered to continue to transmit support payments received from the Respondent
~ to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notfce to Discontinue Payments from the Department
~ of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
