THIS CAUSE having come on for hearing and all parties havi~ received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERED AND ADJUDGID-that the minor child(ren)
EBONY AISHA PRIDE, d.o.b. 12-6-78
is/are declared to be the legitimate child(ren) of t(ie Defendant
ERIC EUGENE GOLLETTE, JR. , and PRISCILLA D. PRIDE
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother, '
PRISCILLA D. PRIDE shall have custody of the said child(ren)
subject to the Defendant's right of reasonable visitation; it is further
ORDERED AND ADJUDGID that commencing on _/~L~t~ ~ 1979,
the Defendant/Father shall pay child support for and on b alf of the said child(ren)

Said amount shall be remitted monthly by the Clerk to the De artment of Healt a•

Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further .
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Itealth and Rehabilitative Services, in which event the support
~ payments shall thereafter be directed and payable to the aforesaid natural mother
or person raving custody of the child(ren); it is further
ORDERID AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF 1{EALTi! AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to.
