• THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AIgD REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party'by virtue of_•an assignment of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further

Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, _
