THIS CAUSE having come on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
RICHARD LEE FARNELL, JR., d.o,b. 12-8-78 -
is/are declared to be the legitimate child(ren) of the Defendant
RICHARD LEE FARNELL, SR. and THERESA ELAINE ROLLE
the natural mother; it is further .
ORDERED AND ADJUDGED that the natural mother,
THERESA ELAINE ROLLE shall have custody of the said child(ren)
subject to the Defendant's right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on ` ~U 1979,
the Defendant/Father shall pay child support for and on behal of the said chi d,(r
- until $219.00 AFDC arrears are pa~ in ~1
in the amount of $ 30.00 per month/ plus $2.00 statutory fee. All
payments shall be made in cash, money order or cashiers check. All money orders, and
cashiers checks shall bear the payees name and Social Security Number and shall be
made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Iiealth and
Rehabilitative Services,'Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
~ hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of 1lealth and Rehabilitative Services, in which event the support
q payments shall thereafter be directed and payable to the aforesaid natural mother
or person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
- the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to.

1. amend the above-named child's/children's birth certificate(s) to show the
above-named father's name.
2. remove from the above-named child's/
children's birth certificate(s) and enter the above-named father's name.
