THIS CAUSE having come on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child GUERIAN VAHNDRELL DESHOND
HALL, d.o.b. 8-15-74, is declared to be the legitimate child of the Defendant
CHARLES WALTER HALL and DEBRA JEAI~IETTE TRAVIS, the natural mother; it is further
ORDBRID AND ADJUDGED that the natural mother, DEBRA JEANETTE TRAVIS,
shall have custody of the said child subject to the Defendant's right of reason-
able visitation; it is further
ORDERED AND ADJUDGED that the Defendant/Father shall pay child support
for and on behalf of the said child in the amount of $12.50 per week, plus $2.00
statutory fee, when he starts working or when he begins receiving government funds
for his disability. Al1 payments shall be made in cash, money order or cashiers
check. All money orders and cashiers checks shall bear the payees name and Social
Security number and shall be made payable to the CLERK OF THE CIRCUIT COURT, and
sent to:

~ Said amount shall be remitted monthly by the Clerk to the Department of Nealth and

~ Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
' Tallahassee, Florida, 32304; it is further
i ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and he is
~ hereby ordered to continue. to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
3 the Department of Health and Rehabilitative Services, in which event the support
~ payments shall thereafter be directed and payable to the aforesaid natural mother
or person having custody of the child; it is further
~ ORDERID AND ADJUDGED that the above-named Defendant having been
~ ad~udicated the father of the above-named child, the DEPAR~iENT OF HEALTH AND
REHABILITATIVE SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it
is hereby ordered to amend the above-named child's birth certificate to show the
above-named father's name.
