The undersigned, as person~l representative of the above estate alleges:
1. The decedent, EDWARD Tc)MAINO , a resident of
St. Lucie County Florida , died on

Petitioner on December 21 , 19 78 .
2. Petitioner Cles herewith a final accounting containing a co~nplete report of al! receipts
and disbursements since the commencement of administration of this estate, or since the date of the
last uccounting fded herein, if any.
3. Petitioner has fully administered this estate, by making payment, settlement, or other _3
d'uposition of all claims and debts that were presented, and by paying or making provision for the

4. Petitioner has filed all required estate tax returns with the lnternal Revenue Service and

with the Department of Revenue of the State of Ftorida, and has obtained and filed with this court
i evidence of the satisfaction of this estate's obligations for both federal and Florida estate taxes, if any.
