COMES NOW, HOME INSURANCE COMPANY, the workmen's 'compensation
insurance carrier for the EVANS PACKING COMPA*IY, the employer of the
Plaintiff at the time of the accident which is the subject matter
of this suit, and files this its lien for its share of the proceeds
of any settlement or judgment rendered in favor of the
Plaintiff in this cause and as grounds therefor would show that
it has paid both disability benefits and medical benefits to
the claimant and hereky claims this lien.
