AFFIDAVIT OF OPERATORS USING TRADE NAME
STATE OF FLORIDA .
COUNTY OF ST. LUCIE
BEFORE ME, the undersigned authority, this day personally
appeared JOSEPH A. BOCCI and LEE BOCCI, who being by me first duly sworn,
deposes and says:
That this Affidavit is made in compliance with-Section
85.09 of the Florida utatutes. "
That the Affiants compose all of the persons interested in
the business enterprise known and doing business as: J S L ENTERPRISES,
located and operating at the following address: 3190 Gran Via Way, Stuart,
Florida 33494, and that the extent-of the interest of each Affiant in said
