THIS CAUSE having come on for hearing and all parties having received
proper and timely notice; the Court having heard testimony and/or considered the
pleadings, papers, affidavits and other papers filed herein, and being otherwise
fully and well advised in the premises, it is
ORDERED AND ADJUDC® that the minor child(ren)
ALBERT MORRIS ANDERSON, JR., d.o.b. 5-6-69
is/are declared to be the legitimate child(ren) of tine Defendant
ALBERT MORRIS ANDERSON and DOROTHY McMILLAN
the natural mother; it is further -
ORDERED AND ADJUDGED that the natural mother,
~IIBLITILY McMILLAN shall have custody of the said child(ren)
subject to the Defendant's right of reasonable visitation; it is further
ORDERID AND ADJUDGED that commencing on ~y K ~ L 'L_ 1979,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)

Rehabilitative Services, Child Support Enforcement Unit, 1317 ilinewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of llealth and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother
r< or person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMEPIDMENT UNIT, shall and it is hereby ordered to:
