PETITION FOR DISCHARGE
The undersigned, as personal representative of the above
Estate, alleges:
1. The decedent, HUGO LEVI, a resident of St. Lucie County,
Florida, died on May 5, 1978, and Letters of Administration were
issued to Petitioner on July 12, 1978.
2. Petitioner files herewith a final accounting containing a
complete report of all receipts and disbursements since the commence-
ment of administration of this Estate. .
3. Petitioner has fully administered this Estate by making
payment, settlement, or other disposition of all claims and debts that
were presented, and by paying or making provision for the payment of
all taxes and expenses of admir~istration.

! 4. The amount of compensation either paid or to be paid to

~ the personal re~resentative, attorneys, accountants, appraisers, or
~ other agents employed by the personal representative is set forth in
Exhibit A attached hereto. -
5. Petitioner has filed all required Estate tax returns with
the Internal Revenue Service and with the Department of Revenue of the
State of Florida, and has obtained and filed with the Court evidence
of the satisfaction of this Estate's obligations for both Federal and
Florida Estate taxes, if any.

~ 6. Petitioner has nade or proposes to make distribution of

~ the assets of this Estate as set forth in the Schedule of Distribution
attached hereto as Exhibit B.
