THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
~ OF HEAI.TH AND REHABILITATIVE SSRVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assig~ent of rights to
child support signed by the party having custody of the dependent chiid(ren) and
all parties having received proper notice, it is
O1tDERF•D AND ADJUDCF.D that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDER~ AND ADNDGID that co~encing on ~'uLi~~?~ , 1979,
the responsible parent WILSON HARDII~(AN, JR. , who is the father

of the minor child(ren), to-wit: AARON LaSHON HARDEMAN, d.o.b. 11-1-77
shall pay to the Clerk of this Court the sum of O.C~'O per 4J Ce each and ~
every" G' , plus $2.00 statutory fee. All payments shall be made in cash,
money order or cashiers check. Al1 money orders and cashiers checks shall bear the
' payees name and Social Security I~umber and shall be made payable to the CLERK OF THE

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 i,Tinewood Boulevard,
Tallahassee, Florida, 32304. It is~further
ORDERED AND ADJUDGF.D that the Clerk of the Circuit Court shall and (s)he
~ is hereby ordered to contiaue to transmit support payments received from the Respoadent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice ta Discontinue Payments from the Department
of Health and Rehabilitative Services, in which event.the support payments shall there-
after be directed and payable to the peraon having custody of the child(ren).
