IT IS THEREFORE ORDERED AND DECREED that Petitioner be and is
hereby appointed Possessory Conservator of:

and each Possessory Conservator shall have the right to visit with 'said
child(ren) at all times mutually agreeable to both parties.
IT IS FURTHER ORDERED AND DECREED that each Possessory Conservator
shall have the rights, privileges, .duties and powers during the period.

The Court, having considered the circumstances of the parties, `
finds that Petitioner is obligated to support the child(ren) over

for each of said child(ren) vn the 5th day of each month, beginning
