2. Petitioner, whose date of birth was 6/29/16, is 63 years of age,
and her social security number is 050-14-9123.
3. Petitioner, although mentally competent, is incapable of the
care custod ~
y, and management of her estate by reason of age or physical infirm-
ity and voluntarily petitions the court for the appointment of a guardian of
her property pursuant to the applicable provisions of the Florida Statutes.
4. This petition is accompanied by a certificate of a licensed
physician that he has examined petitioner and that petitioner is competent to
understand the nature of the guardianship and her delegation of authority.
5. The approximate value and description of the petitioner's
property are: Personal and Real Property, Savings Bonds, Cash and Patents.
Approximate value is less than $60,000.00.
