Rehabilitative Services, Child Support Snforcewent Unit, 1317 Hinewood Boulevard,
Tallahassee, Florida, 32304; it is further .
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Iiotice to Discontinue Paywsnts from
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother
or_ person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-Hawed Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIYL
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:

1. amend the above-named child's/children's birth certificate(s) to show the
above-named father's name.
2. remove Eros the above-named child's/
children's birth certificate(s) and enter the above-named father's name.
