proper and timely notice; the Court having heard testissony and/or considered the

Said assount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winetiood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Pay~oents from _
the Department of Health and Rehabilitative Services, in eltich event the support
. payments shall thereafter be directed .and payable to the aforesaid natural mother
or person having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-na~ued Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES., BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
