ANGELIA SABRINA STOKES, d.o.b. 11-24-76; JULIUS AUGUSTUS STOKES,. d.o.b. 2-3-78 is/are declared to be the legitimate,child(ren) of the Defendant DAVID EUGENE STOKES ~ ,and ANNETTE BELL - the natural ~aother; it is further ORDERED AND ADJUDGED that the natural pother, ANNETTE BELL ,shall have custody of the said child(ren) sub3ect to the Defendant s right of reasonable visitation; it is further ORDERED AND ADJUDGED that conuoencing on AUGUST 28 , 1979, the Defendant/Father shall pay child support for and on behalf of the said child(ren) in the amount of $ 20.00. per week ,plus $2.00 statutory fee. All ~ payments shall be made in cash, awney order or cashiers check. Said amount shall be remitted monthly by the Clerk to the Department of Health aod_ Rehabilitative Services, Child Support Enforcement Unit, 1317 Rinewood noulevasd, Tallahassee, Plorida, 32304; it is further - ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is hereby ordered to continue to transmit support payments received from the Defendant until further order of this Court or receipt of Notice to Discontinue Payvents ~rao the Department of Health and Rehabilitative Services, in which event the wpport payments shall thereafter be directed and payable to the aforesaid natural ~othet or person having custody of the.ch3ld(ren); it is further ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated the father-of the above-named child(ren), the.DEPARTMENT OP HEALTH AND REHABILITATIVE SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to: (Check applicable paragraph) xx 1. amend the above-named child's/children's birth certificate(s) to show the above-named father's name. 2. remove from the above-named child's/ children's birth certificate(s) and enter the above-named father's name.
